Description The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple comorbidities. This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients. Looking for RN Care Manager local to Pierce, Mason, Thurston, Lewis, Cowlitz counties in Washington.   • Graduation from an accredited school of nursing.
  • Upon hire: Washington Registered Nurse License
  • 2 years of work experience in RN role. Preferred qualifications:
    • Bachelor's Degree in Nursing.
    • Experience managing complex patients in an ambulatory setting.
