Job Description:
Description The Diabetes Education Specialist plays a vital role
in empowering patients and families to successfully manage diabetes
through high-quality, evidence-based education. This position
delivers culturally relevant, age-appropriate diabetes
self-management and nutrition education in both individual and
group settings. Serving as a trusted clinical and educational
resource, the Diabetes Education Specialist collaborates closely
with patients, families, healthcare professionals, and the
community to support long-term health outcomes. The role also
contributes to program development, quality improvement
initiatives, and the delivery of continuing education to
professionals and the public. Key Responsibilities: - Provide
comprehensive diabetes self-management education and evidence-based
nutrition guidance to patients in individual and group settings,
using culturally relevant and age-appropriate approaches. - Assess
the educational needs of patients and their families, develop
individualized education plans, implement those plans, and evaluate
outcomes, revising strategies as needed. - Serve as a clinical and
educational resource for patients, the community, and healthcare
professionals both within and outside the Center. - Coordinate
ongoing patient care and support in collaboration with appropriate
members of the interdisciplinary care team. - Deliver education,
training, and ongoing support related to diabetes technologies to
enhance patient understanding and self-management. - Plan and
conduct continuing education programs in diabetes and general
nutrition for allied health professionals and the general public. -
Participate in the assessment, planning, implementation, and
evaluation of the Center’s diabetes education program, including
continuous quality improvement initiatives. - Support activities
related to maintaining recognition with the American Diabetes
Association, ensuring adherence to program standards. - Demonstrate
strong written and verbal communication skills in interactions with
patients, families, colleagues, and external partners. Providence

them. Required qualifications: - Bachelor’s degree in Nutrition
Sciences or Dietetics - Washington Dietitian Certification upon
hire or Washington Nutritionist Certification upon hire - National
CDR Registered Dietitian - Commission on Dietetic Registration upon
hire - Certified Diabetes Care and Education Specialist (CDCES)
within 1 year of hire - 4 years of nutrition experience, preferably
one year in ambulatory care setting or community health - 1000
hours of diabetes education experience in a variety of settings -
Experience and comfort with teaching and public speaking Preferred
qualifications: - Master's degree in Nutrition Sciences or
